  • How do I find a doctor or hopital in my area in-network?

    Go to the Horizon website www.horizonblue.com. Click on members and individuals. Click, On Line Provider Directory or Quick Physician or Dentist search. Enter PPO for your plan.

    How do I download a claim fom?

    Go to the Horizon website www.horizonblue.com.  Click on members and individuals. Go to Forms and News Letters. Then click down loadable forms. Click Traditional forms. Dental forms, click Dental services and downloadable forms. Click Dental Service Report (claim form) for NJ 7902.

    How can I see my processed claims online?

    Go to the Horizon website www.horizonblue.com. Sign up and get your own pin number. Logon to see processed claims.

    Who can I call if I have a problem with a claim?

    You can call the customer service line at Horizon Blue Cross Blue Shield at 1-800-355-2583 or the Fund office at 973-473-5544. 

    How do I enroll for mail order prescription drugs?

    Go to the IPS website www.ipsrx.com and click member info.

    How do I get reimbursed for drugs I paid for?

    Go to the Envision Rx website www.envisionrx.com and click resource tools. Click direct member reimbursement.
